At Optum, we're dedicated to delivering high-quality care to our patients. As a Home Infusion Nurse, you'll play a critical role in providing clinical care and education to patients receiving parenteral medications in the comfort of their own homes.
Key Responsibilities:
- Establish and maintain positive relationships with patients and their families, providing education on self-care and self-administration of medications.
- Assess patient appropriateness for home infusion therapy, ensuring a safe and supportive environment.
- Obtain and document patient medical history, including diagnosis and treatment plans.
- Complete all required clinical documentation, including nursing plans of care and visit notes.
- Participate in case conferences to develop comprehensive care plans with the multidisciplinary team.
- Provide skilled care to patients of varying ages and medical conditions, following physician orders and treatment plans.
- Establish and maintain vascular access devices, including peripheral IV placement and dressing changes.
- Administer medications as prescribed, monitoring patient response and side effects.
- Communicate with the care team to ensure seamless patient care and support.
Requirements:
- Active and unrestricted licensure as a Registered Nurse (RN) in the state(s) of practice.
- Current Basic Life Support (BLS) and/or CPR certification.
- 1+ years of experience working as an RN in a clinical care setting, with proficiency in infusion therapy.
- Proficient with electronic medical record documentation.
- Ability to work independently in home or alternate-site settings.
- Flexibility to meet attendance, overtime, and after-hours on-call requirements.
Preferred Qualifications:
- Experience administering infusion therapy in a home or alternate-site setting.
- Knowledge of Microsoft Office and electronic medical records (EMR) programs.
- Knowledge of the Infusion Nursing Society (INS) standards of practice, policies, and procedures.
- Proven excellent organizational and critical thinking skills.
- Effective interpersonal communications, both written and verbal.
