Primary Care Physician Leadership Opportunity

An exciting opportunity awaits a seasoned primary care physician to shape the future of healthcare delivery across four counties. As Medical Director of Primary Care & Population Health, you will play a pivotal role in influencing health care strategies and creating positive change for physicians and patients.
About the Role- Drive clinical performance regionally to achieve favorable patient outcomes.
- Collaborate with diverse specialties and disciplines to develop integrated care pathways.
- Foster a culture of trust, collaboration, and innovation among primary care clinicians serving diverse populations.
- Maintain a part-time clinical practice at one of our established sites to stay connected with the primary care workforce.
- Board Certification: Family Medicine, Internal Medicine, Pediatrics, or Geriatrics.
- At least 2 years of primary care leadership experience in a system-based role.
- Strong understanding of population health strategies, including patient empanelment, activation, disease management, care management, risk stratification, and medication therapy management.
- Familiarity with clinical guidelines and evidence rating methodologies.
- Proficiency in metric report configuration and validation.
- Excellent clinical skills and strong communication abilities.
The estimated annual salary for this position is $250,000 - $300,000, based on industry standards and location considerations.
