Medical Director of Primary Care and Population Health in Annapolis Area

Physician Empire seeks a Medical Director to lead our primary care workforce in the Annapolis area. This progressive position is ideal for experienced primary care physicians looking to drive strategic decision-making and create positive change.
About the RoleThis full-time position will involve leading a team of primary care clinicians across four counties, driving clinical performance to produce favorable patient outcomes, and participating in integrated care pathways with other specialties and disciplines.
Key Objectives- Lead a primary care workforce prepared for payment reform and care redesign opportunities in Maryland.
- Drive primary care clinical performance regionally to produce favorable patient outcomes.
- Participate in the development of integrated care pathways with other specialties and disciplines.
- Foster an environment of trust, collaboration, and innovation among diverse primary care clinicians serving diverse populations.
- Maintain a part-time clinical practice at one of our established sites to experience first-hand the challenges and opportunities for the primary care workforce.
- Board Certification in Family Medicine, Internal Medicine, Pediatrics, or Geriatrics.
- At least 2 years of primary care leadership experience in system-based roles.
- Fluency in population health strategies, including patient empanelment, patient activation, disease management, care management, risk stratification, and medication therapy management.
- Familiarity with clinical guidelines and evidence rating methodologies.
- Familiarity with metric report configuration and validation.
- Excellent clinical skills.
- Excellent writing and public speaking skills.
