**Job Overview**

Sun River Health is seeking a full-time Patient Care Coordinator to support our Haverstraw, NY site.

The Patient Care Coordinator identifies individuals and/or members of their families within the target population who are in need of preventive and primary health care services and links them to the services they need.

Responsibilities include consulting with medical staff, facilitating patient appointments, assisting patients in overcoming obstacles, and tracking progress.

The ideal candidate will have a high school diploma or equivalent, 2-4 years of work experience, and strong communication skills.

Bilingual Spanish is preferred.

**Salary and Benefits**

The estimated annual salary for this position is $45,760 - $49,920 based on an hourly rate of $22.00 - $24.00.

Job Responsibilities

  1. Consult with medical staff to link patients to services through the provision of ancillaries such as translation, transportation assistance and application assistance.
  2. Facilitates patient appointments within different departments of SRH and outside agencies.
  3. Assist patient in overcoming obstacles that would prevent them from receiving needed referrals.
  4. Responsible for facilitating patients to appropriate referred agencies, with specific focus on referrals to specialty care and public health insurance programs, as needed and appropriate; track progress in eCW and alert providers.
  5. Answer patient questions to clarify provider instructions, procedures and referral needs.
  6. Responsible for transmitting all necessary clinical information available to specialist for referral; obtains consultant reports; obtains consent for release of information.
  7. Review open referrals for upcoming medical appointments and provide to clinical team.
