Clinical Social Work Professional

Seeking an experienced Clinical Social Work Professional to join our interdisciplinary team at Saint Francis Health System.
About the Role:
We are looking for a skilled Clinical Social Worker to provide comprehensive social work services in areas of case management, discharge planning, and continuing care. The ideal candidate will have experience working in hospital settings and be proficient in handling crisis situations.
Responsibilities:
- Assess patients and families referred to social service work and develop individualized plans for care.
- Collaborate with healthcare professionals to integrate social work plans into overall patient care.
- Conduct mental health assessments, complete legal paperwork, and provide safety planning for patients in crisis.
- Provide education and resources to patients and families on social determinants of health.
- Develop and implement specialized knowledge of resources related to patient populations.
- Document patient care plans and interventions within the medical record.
- Complete statistical reports as required.
Requirements:
- Master's degree in Social Work or related behavioral health field.
- Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC).
- Minimum 5 years of experience in hospital or community health setting.
Work Environment:
This is a full-time position at our Tulsa, Oklahoma location. As a member of our team, you will have opportunities for professional growth and development.
Compensation:
The estimated salary for this role is $65,000 - $85,000 per year, depending on experience.
