Job Summary
The Clinical Pharmacist at Northwestern Memorial Healthcare reflects the mission, vision, and values of the organization, adhering to the Code of Ethics and Corporate Compliance Program, and complying with all relevant policies, procedures, guidelines, and regulatory standards.
Key Responsibilities:
- Interprets medication orders to ensure safety and proper use, monitoring for compatibilities, incompatibilities, medication duplication, allergies, and potential administration errors.
- Coordinates medication-related problems between pharmacy, nursing, and prescribers, demonstrating initiative and sound judgment.
- Monitors medication therapy for significant drug-drug, drug-diet, or drug-laboratory test interactions, intervening when necessary.
- Evaluates and documents the appropriateness of medication selections for desired therapeutic outcomes.
- Communicates potential problems and solutions to prescribers.
- Acts as a consultant in pharmacokinetics and adverse reactions to healthcare professionals.
- Participates in therapeutic drug monitoring or intervention programs, adjusting therapy as needed.
- Processes medication order information into the Electronic Health Record for accuracy.
- Maintains comprehensive pharmacy patient profiles, including age, maturation, weight, allergies, diagnosis, and pharmacokinetic parameters.
- Oversees medication packaging, labeling, and distribution according to established policies and guidelines.
- Monitors scheduled medication use and maintains controlled inventories.
- Compounds and dispenses pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products.
- Verifies medication orders for accuracy and completeness, resolving discrepancies as necessary.
- Reviews laboratory data for appropriateness of medication use, recommending changes when needed.
- Develops and presents education and training programs for staff and assists in educating pharmacy students, residents, and fellows.
- Provides accurate and timely medication information and teaching to healthcare professionals and patients.
- Maintains the hospital formulary and limits medication usage to formulary items when appropriate.
- Documents clinical activities and interventions performed.
- Collects data, maintains logs, and records as required by the department.
- Communicates professional knowledge effectively in oral and written forms to patients and healthcare professionals.
- Participates in quality improvement and medication use review activities.
- Organizes, prioritizes, and plans workload within given time constraints.
- Works cooperatively with fellow pharmacists to oversee and direct work area activities.
- Reviews and resolves issues with automated dispensing cabinets and ensures compliance with hazardous waste procedures.
- Maintains proficiency and expertise in areas of practice through continuous professional development.
Qualifications:
- Current license to practice pharmacy in the State of Illinois.
- Bachelor's Degree in Pharmacy or Doctor of Pharmacy Degree.
Preferred Qualifications:
- Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.).
- Completion of a pharmacy residency program or equivalent hospital pharmacy experience.
