Aveanna Healthcare is seeking a highly skilled and experienced Hospice RN Clinical Director to join our team. As a key member of our leadership team, you will be responsible for overseeing the planning, coordination, and delivery of quality care services to our clients.

Key Responsibilities
  • Leadership and Oversight
    • Communicate with primary physicians to evaluate total patient/family needs and establish the Hospice Plan of Care within 24 hours of admission.
    • Oversee the assignment of each patient/family to appropriate field staff within 24 hours of admission.
    • Coordinate the delivery of patient/family care with the interdisciplinary team members, including the Medical Director and attending physician.
  • Interdisciplinary Team Management
    • Provide for regular interdisciplinary team meetings for the development, review, and revision of the patient/family plan of care, including exchange of information, review of problems, assessing effectiveness of interventions, and documentation of outcomes.
    • Ensure the timely and adequate delivery of Hospice services to patients and families through ongoing evaluation of interventions, goals, and outcomes.
  • Staff Supervision and Development
    • Ensure new staff orientation to job duties by assisting in the orientation plan, providing ongoing supervision, and evaluation, establishing preceptors as needed.
    • Identify need for staff education and assist in the development of in-service programs.
  • Quality Assurance and Compliance
    • Supervise direct care delivery activities of team members for conformity to established standards of care as set forth by the Standards of Hospice Nursing Practice, Medicare regulations, Joint Commission standards, and other professional licensing bodies.
    • Review patient care documentation, oversee ongoing audit of patient records, resolution of deficiencies, and routinely review audit findings.
  • Additional Responsibilities
    • Oversee the scheduling of 24-hour nursing coverage for hospice patients.
    • Promote quality-oriented and cost-effective utilization of hospice resources.
    • Makes visits to patients/families as indicated by patient/family need, staffing resources, and supervisory requirements.
    • Participate in recruitment and selection of professional and non-professional field staff, perform probationary and annual performance reviews of supervised staff.
    • Participate in on-call rotation for delivery of care, after office hours, on weekends, and holidays.
    • Maintain sound privacy and security practices and prevent privacy or security breaches. If breach occurs, take corrective action.
    Requirements
    • Current license as a Registered Nurse in the State of Application.
    • Valid CPR certification.
    • Valid Driver's License.
    • Minimum of two years of supervisory experience in a hospice setting.
    • Demonstrated ability to manage all aspects of terminal care, including family counseling and support, pain and symptom management, and bedside nursing.
    • Has a thorough knowledge of Medicare regulations, Joint Commission standards governing the delivery of Hospice services.
