Clinical Liaison Expert

Job Description: As a Clinical Manager at Cottage Hospital, you will provide patient consultation, assistance, and education about VNA Health services under the direction of the COO. You will evaluate, plan, and coordinate patient referrals, develop relationships that promote business development, and support the mission, values, and vision of VNA Health.
Key Responsibilities:
- Collaborate with referral sources and the Community Liaison team to facilitate referrals, meet admission goals, and ensure timely response.
- Evaluate potential patients for eligibility and appropriateness of home health, hospice, or personal care services.
- Consult with physicians, discharge planning personnel, and insurance Case Managers to facilitate safe discharges and coordinated home plans of care.
- Coordinate referrals with agency contacts, establish communication lines between VNA Health and referral sources, and provide ongoing education to referral sources and the community.
Requirements: Current clinical license (RN, LVN) in California, CPR certification, ability to develop and maintain positive relationships, promote services, and grow business, excellent observation and communication skills, and experience in home care and hospice regulations and insurance benefit eligibility highly desired.
