About the Role

We are seeking a skilled and compassionate Registered Nurse to join our team as a Triage Registered Nurse. As a key member of our care team, you will be responsible for providing exceptional patient care and support to our patients and their families.

Key Responsibilities
  • Assess patient needs and provide guidance over the phone
  • Coordinate care with the care team, facility, and patient's caregiver
  • Schedule home visits as needed
  • Use judgment, knowledge, and experience to ensure patients receive proper and timely care
Requirements
  • Graduate of an accredited nursing school (BSN preferred)
  • Current state license as a registered nurse
  • Minimum three years of experience as a registered nurse in an appropriate clinical care setting, home health, or hospice environment (preferred)
  • Two years of hospice experience required
