Job Description
We're seeking an experienced Registered Nurse to join our team as a Care Coordinator. As a key member of our healthcare team, you'll be responsible for developing personalized care plans, advocating for residents, and ensuring high standards of care and safety.

Key Responsibilities:
* Develop and implement individualized care plans for each resident based on their unique needs and preferences
* Collaborate with the healthcare team to ensure seamless care delivery
* Advocate for residents' rights and dignity, promoting a culture of respect and compassion
* Monitor and report any changes in residents' conditions or concerns

Required Skills and Qualifications
* Current RN licensure in good standing
* 2+ years of experience in long-term care or a related field
* Strong knowledge of nursing practices, laws, and regulations
* Excellent communication and interpersonal skills
* Ability to work collaboratively in a fast-paced environment
