About the Role

We are seeking a skilled Social Services Coordinator to join our team at Regency at Fremont. As a Social Services Coordinator, you will play a vital role in providing medically-related social services to our residents, helping them to attain and/or maintain their physical, mental, and psychosocial well-being.

Key Responsibilities
  • Complete admissions, social services evaluations, and provide referrals to residents and their families.
  • Assist residents with financial and legal matters, such as applying for pensions, referrals to lawyers, and preplanning arrangements with funeral homes.
  • Provide discharge planning services, including helping residents transition to community congregate living, arranging home care services, and assisting with transfer arrangements to other facilities.
  • Provide or arrange needed counseling services and psychiatric medication management as indicated.
  • Ensure accurate and timely documentation in MDS care plans.
Requirements
  • Current social work licensure in the state.
  • Bachelor's degree in social work required; master's degree preferred.
  • One year of supervised social work experience in a healthcare setting is preferred.
