We are seeking a skilled and compassionate Registered Nurse to join our team at Caledonia Home Health Care and Hospice, a division of Northern Counties Health Care. As a Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities:

  • Provide comprehensive nursing care to patients in their homes, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

Requirements:
