Position Overview

We are seeking a dedicated Registered Nurse Care Coordinator to join our esteemed team at Bishop Gadsden. Our organization is committed to delivering compassionate and holistic care to our residents.

Key Responsibilities
  • Facilitate all requests for resident Care Navigation services.
  • Guide residents through their healthcare decisions in a nursing capacity.
  • Collaborate with the Care Navigator to address resident needs efficiently.
  • Schedule and manage appointments flexibly with the Care Navigator.
  • Implement and monitor short and long-term care plans to ensure emotional and mental well-being.
  • Ensure timely arrangement of necessary resources for appointments.
  • Document and submit billing for services rendered on a weekly basis.
  • Communicate resident concerns effectively to the Care Navigator.
  • Act as a liaison for family members regarding billable care needs.
  • Transport residents to various appointments as needed.
  • Maintain safety protocols for residents with diverse cognitive and functional abilities.
  • Assess and report on levels of care or transfer needs to the Care Navigator.
  • Conduct assessments for new clients within our home care service.
  • Complete necessary documentation for Long Term Care insurance clients.
Qualifications

The ideal candidate will possess:

  • Proficiency in computer applications.
  • Knowledge of senior services and resources.
  • Strong verbal and written communication skills.
  • A compassionate approach towards residents and their families.
  • Basic computer skills, including familiarity with Microsoft Office programs.
