Case Manager/Family Development Specialist

Family Service League Inc. is seeking a dedicated and compassionate Case Manager to join our team in Bay Shore. As a key member of our Home Base program, you will provide direct services to youth and families, ensuring they receive the support and resources they need to thrive.
Responsibilities- Conduct linkage, coordination, advocacy, and mentoring to support youth and families in achieving their goals.
- Provide anger management and parenting skills training to youth and their families, promoting healthy relationships and communication.
- Focus services on goal achievement surrounding the factors that contributed to the youth's involvement with mental health services.
- Carry a caseload of 14 youth, visiting them weekly to provide ongoing support and guidance.
- Refer clients as needed for skill building services and work collaboratively with the skill builder to achieve client goals.
- Conduct home visits throughout Suffolk County, ensuring a comprehensive approach to service delivery.
- High school diploma or equivalent required; Bachelor's degree in social work or related field preferred.
- At least 2 years of experience working in human services, with a proven track record of working with diverse populations.
- Excellent interpersonal and verbal and written communication skills, with the ability to engage both youth and parents in service delivery.
- Knowledge of community resources, sound judgment, reliability, flexibility, and the ability to multitask are essential.
- Proficient computer skills, including Microsoft Office, and a valid and clean NYS Driver's License are required.
