Key Responsibilities:

  • Provide comprehensive diabetes self-management education and support to patients and their families.
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans.
  • Conduct patient assessments, develop educational plans, and provide educational interventions.
  • Liaise with community partners and healthcare providers to optimize patient care.
  • Participate in quality improvement initiatives to enhance diabetes care delivery.

Requirements:

  • Graduate from an accredited school of nursing or Bachelor's degree in Food and Nutrition.
  • Three years of clinical experience in direct care of people with diabetes and associated conditions.
  • Current, valid RN license or temporary RN license from the Texas Board of Nursing.
  • Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE) certification.

Preferred Qualifications:

  • Bilingual (oral and writing) in Spanish.
  • Basic computer skills with working knowledge of database programs, data report analysis software, and electronic medical record charting.
