Patient Health Navigator

Sea Mar Community Health Centers is seeking a highly motivated and compassionate Patient Health Navigator to join our team. As a Patient Health Navigator, you will play a vital role in ensuring that patients receive high-quality, patient-centered care. Your primary responsibility will be to navigate patients through the healthcare system, providing support and guidance to help them access necessary medical services.
Key Responsibilities- Schedule Appointments and Coordinate Care
Work closely with patients, healthcare providers, and other stakeholders to schedule appointments and coordinate care. Ensure that patients receive timely and effective treatment for their medical needs.
Provide Patient Support and EducationOffer emotional support and education to patients and their families, helping them to understand their medical conditions and treatment options. Provide guidance on healthy lifestyle choices and disease management.
Facilitate Communication and CollaborationAct as a liaison between patients, healthcare providers, and other stakeholders, ensuring that communication is clear and effective. Facilitate collaboration among healthcare teams to ensure that patients receive comprehensive care.
Identify and Address Health DisparitiesWork to identify and address health disparities among patient populations, developing strategies to improve health outcomes and reduce health inequities.
Participate in Quality Improvement InitiativesContribute to quality improvement initiatives, helping to identify areas for improvement and develop strategies to enhance patient care.
Requirements- Education and Experience
A high school diploma or equivalent is required. Previous experience in a healthcare setting or patient advocacy role is preferred.
Communication and Interpersonal SkillsExcellent communication and interpersonal skills are essential for this role. You must be able to work effectively with patients, healthcare providers, and other stakeholders.
Cultural CompetenceCultural competence is critical for this role. You must be able to work effectively with diverse patient populations and understand the cultural nuances that impact healthcare.
Language SkillsBilingual English/Spanish language skills are preferred, but not required.
