The Community Youth Support Specialist role is dedicated to assisting young individuals through the DCS program, focusing on the development and maintenance of essential life skills that improve their overall well-being within the community.
Key Responsibilities:
- Provide personal care and support in daily routines, helping youth achieve their individualized care plans.
- Embrace and embody Turning Point's core values and philosophy in all interactions with youth and community members, utilizing respectful communication and connecting them to preferred activities and experiences.
- Facilitate community integration for youth, ensuring they feel included, respected, and valued while fostering diverse relationships in their chosen community.
Transportation and Safety:
- Ensure safe transportation for youth to various events, educational institutions, and medical appointments, using agency vehicles, personal vehicles, or public transport as necessary.
- Manage alternate transportation arrangements when safety concerns arise with unpaid support.
Resource Management:
- Oversee the appropriate use of personal and agency resources, including funds, vehicles, and supplies.
- Attend all required training sessions provided by Turning Point or DCS.
Nutritional Support:
- Prepare and serve meals according to scheduled menus, ensuring that nutritional needs are met.
Skill Development:
- Engage in daily activities or instruct youth in developing daily living skills and personal competencies aimed at fostering independence.
- Document progress consistently towards achieving set outcomes.
Record Keeping:
- Utilize electronic record systems to complete daily shift notes as mandated by funding entities.
Stakeholder Engagement:
- Promote customer satisfaction through regular communication with youth, families, DCS, and external partners.
Medication Management:
- Supervise the storage and administration of medications in accordance with established standards.
Personal Care:
- Ensure youth maintain personal hygiene with as much assistance as they can provide.
- Maintain cleanliness of the living environment and assist youth with laundry tasks.
Vehicle Maintenance:
- Ensure that transportation vehicles are clean and properly maintained.
Faith and Confidentiality:
- Support youth in participating in their chosen religious or spiritual practices.
- Uphold the confidentiality of all youth information.
