Job Title: Home Care Registered NurseJob Summary:

Trinity Health Of New England At Home is seeking a compassionate and skilled Home Care Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Care RN, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing education and support to patients and their families.

Key Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed.
  • Monitor patient progress, adjusting care plans as needed to ensure optimal outcomes.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements:
  • Graduate of an accredited nursing program, BSN preferred.
  • Licensure as a Registered Nurse in the State of Connecticut.
  • At least one year of experience as a professional acute care nurse, with a focus on home health care preferred.
  • Current driver's license and reliable transportation.
