Job Summary:

Trinity Health Of New England At Home is seeking a compassionate and skilled Home Care Registered Nurse, RN to provide primary nursing care to patients in their homes. As a key member of our team, you will be responsible for assessing patient condition and risk, developing and implementing individualized care plans, and providing education and support to patients and their families.

Responsibilities:

Assess patient condition and risk at admission, at required time points, and as needed, utilizing appropriate standardized documentation.
Develop and implement individualized care plans in collaboration with the physician, Population Health Case Manager, and patient.
Provide education and support to patients and their families on disease management, medication, and other health-related topics.
Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.

Requirements:

Graduate of an approved nursing education program, BSN preferred.
Licensure as a Registered Nurse in the State of Connecticut.
One (1) year experience as a professional acute care nurse.
Home care experience preferred but not required.
Must have current Driver's license and reliable transportation.
