Central Counties Services is seeking a highly skilled and dedicated Service Coordinator 2 to join our team. As a Service Coordinator 2, you will play a critical role in providing support and services to individuals with intellectual and developmental disabilities.

Job Summary:

The Service Coordinator 2 will be responsible for assessing the needs of consumers, developing and implementing Person Directed Plans, and monitoring the progress of consumers. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with consumers, families, and other stakeholders.

Key Responsibilities:
  • Assess consumer needs and develop Person Directed Plans
  • Monitor consumer progress and make adjustments to plans as needed
  • Provide support and services to consumers and their families
  • Collaborate with other service providers and stakeholders to ensure comprehensive support
  • Document consumer progress and maintain accurate records
Requirements:
  • Bachelor's degree in a related field (e.g. psychology, social work, etc.)
  • 2 years of experience working with individuals with intellectual and developmental disabilities
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Valid driver's license and reliable transportation
