Salary: $55,200 - $70,400 per yearJob Description

This Clinical Assessment Coordinator position is responsible for responding to inquiry calls, completing initial assessments, and referring patients to appropriate resources. The ideal candidate will possess excellent communication and interpersonal skills, be able to work effectively in a fast-paced environment, and have a high level of emotional intelligence and empathy.

Key Responsibilities:
  • Responds to inquiry calls, completes inquiry forms accurately, and schedules assessments in a timely manner.
  • Conducts assessments at facility or other locations, completes Comprehensive Interdisciplinary Assessments Initial Triage thoroughly.
  • Refers patients who are not appropriate for facility programs to appropriate resources.
