Overview

Catholic Health Initiatives, a leading healthcare provider, is seeking a skilled RN Care Coordinator to join our team. As a RN Care Coordinator, you will play a critical role in overseeing the progression of care and discharge planning for patients requiring these services. Your expertise will ensure that patients receive high-quality, cost-effective care while promoting quality of care and cost-effective outcomes.

Responsibilities

The RN Care Coordinator will be responsible for:

  • Collaborating with utilization management, nursing, physicians, ancillary departments, insurers, and post-acute service providers to progress care toward optimal outcomes.
  • Advocating for patients and families by identifying, valuing, and addressing patient choice, spiritual needs, cultural, language, and socioeconomic barriers to care transitions.
  • Enhancing the patient experience by striving to meet individual health needs.

Qualifications

To be successful in this role, you will need:

  • Minimum two (2) years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of 1 year experience.
  • Graduate of an accredited school of nursing (Bachelor's Degree in Nursing (BSN)) or related healthcare field.
  • At least five (5) years of nursing experience.
