Job Title: Home Health Aide

Community Based Care of Central Florida is seeking a compassionate and dedicated Home Health Aide to join our team.

Job Summary:

We are looking for a skilled and caring individual to provide personal care services to our patients in the comfort of their own homes. As a Home Health Aide, you will work closely with our healthcare team to deliver personalized care tailored to the unique needs of each individual.

Responsibilities:
  • Provide personal care services, including bathing, dressing, and grooming, as needed.
  • Assist with meal preparation and feeding, laundry, and light housekeeping.
  • Follow infection control policies and proper hand washing techniques.
  • Report any observed or reported changes in the client's condition and/or needs to the Registered Nurse.
  • Document care provided and complete the necessary records.
  • Promote personal safety and a safe environment for clients by observing infection control practices and reporting unsafe situations to the Supervisor/Case Manager.
  • Communicate effectively with the interdisciplinary team through verbal reports and participation in staff meetings.
Qualifications:
  • Meet the training and/or certification requirements of the state in which the home health aide services are being provided.
  • Active and verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant (CNA) certification in applicable state, or 1 year of supervised, verifiable experience within the last 3 years.
  • Be at least eighteen (18) years of age.
  • Demonstrated ability to read, write, and follow a written Plan of Care.
  • Good interpersonal skills and a passion for caring for those in need.
