Home Health Care Registered Nurse Job Description

BAYADA Home Health Care is seeking a compassionate and dedicated Registered Nurse (RN) to provide exceptional home health nursing care to adult and geriatric patients in their homes.

Job Summary

We are looking for a skilled and empathetic RN to join our team in providing high-quality home health care services. As a Registered Nurse, you will be responsible for developing and implementing individualized care plans, administering medications, and providing wound care and treatments.

Responsibilities
  • Develop and implement individualized care plans in accordance with patient needs
  • Administer medications, wound care, and treatments as prescribed by physicians
  • Provide education and support to patients and their families on disease management and self-care
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Accurately document patient information and care provided
Requirements
  • Graduation from an accredited and approved nursing program
  • Current RN licensure in the state of employment
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
