As a Home Health Aide, you will play a vital role in enhancing the lives of our patients by providing assistance with daily living activities and ensuring their comfort and well-being.

Responsibilities:
  • Provide personal care services under the direction and supervision of the Registered Nurse
  • Assist with meal preparation and feeding
  • Perform light housekeeping duties
  • Document care provided and complete required forms
  • Report any observed or reported changes in the client's condition and/or needs to the Registered Nurse
  • Communicate effectively with the interdisciplinary team
  • Treat patients and families with respect and maintain confidentiality
Qualifications:
  • Meet the training and/or certification requirements of the state in which the home health aide services are being provided
  • Active and verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant (CNA) certification in applicable state
  • Be at least eighteen (18) years of age
  • Demonstrated ability to read, write, and follow a written Plan of Care
  • Good interpersonal skills and a passion for caring for those in need
