Job Summary

We are seeking a highly skilled Diabetes Care Specialist to join our team As a Diabetes Care Specialist, you will play a critical role in providing comprehensive diabetes education and support to our patients. You will work closely with our multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide diabetes education and support to patients and their families through group classes and one-on-one sessions.
  • Develop and implement individualized care plans that take into account the patient's medical history, lifestyle, and goals.
  • Collaborate with our multidisciplinary team to coordinate care and ensure seamless transitions between healthcare settings.
  • Monitor patient progress and adjust care plans as needed to achieve optimal health outcomes.
Requirements
  • Must be a Registered Nurse (RN) or Registered Dietitian (RD) with experience in diabetes care.
  • Must have strong communication and interpersonal skills to effectively engage with patients and healthcare teams.
  • Must be able to work independently and as part of a team to achieve shared goals.
  • Must have basic computer skills and proficiency in electronic medical record systems.
Salary

$65,000 - $85,000 per year, depending on experience.
