About the Role:

This RN Diabetes Education Consultant position offers a unique opportunity to make a meaningful impact on the lives of patients and their families affected by diabetes. As a member of our team, you will be responsible for providing expert education and support to individuals with diabetes, as well as collaborating with healthcare providers to ensure seamless care coordination.

Responsibilities:
  • Provide comprehensive diabetes self-management education to patients and families.
  • Develop and implement individualized educational plans to meet the unique needs of each patient.
  • Collaborate with healthcare providers to ensure effective communication and care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes.
Requirements:
  • Bachelor's degree in nursing or related field; MSN preferred.
  • Current State of Texas Nursing license; BLS certification required.
  • CDCES or BC-ADM certification within one year of hire.
  • One year of clinical nursing experience preferred.
