Diabetes Care Specialist

We are seeking a highly skilled Diabetes Care Specialist to join our team at Parkland Health and Hospital. As a key member of our healthcare team, you will play a vital role in providing exceptional care to patients with diabetes.
Job OverviewThis is a full-time opportunity for a registered nurse or dietitian who is passionate about delivering high-quality patient-centered care. You will work closely with our multidisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each patient.
Responsibilities- Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
- Provide education and support to patients and their families on self-management techniques, medication management, and lifestyle modifications.
- Liaise with community partners to optimize patient care and outcomes.
- Collaborate with the multidisciplinary team to ensure seamless coordination of care.
- Current RN license or RD certification required.
- Minimum 3 years of clinical experience in diabetes care.
- Bilingual (oral and written) in Spanish preferred.
- Excellent communication and interpersonal skills.
Parkland Health and Hospital offers a competitive salary range of $85,000 - $110,000 per year, depending on experience. Our benefits package includes medical, dental, and vision insurance, as well as a 401(k) retirement plan. We also offer opportunities for professional growth and development.
