Position Overview: This is a full-time position that involves a significant amount of travel within designated areas. The Inpatient Care Coordinator plays a crucial role in overseeing the care of patients during their hospital stay and ensuring a smooth transition to lower levels of care.

Key Responsibilities:

  • Collaborate with an interdisciplinary care team to create personalized transition plans for patients.
  • Act as a clinical liaison with hospital staff and conduct reviews for clinical authorizations based on established criteria.
  • Perform various types of clinical reviews, documenting medical necessity and appropriate care levels.
  • Engage with facility staff and patients to assess discharge needs and formulate effective discharge plans.
  • Utilize population stratification tools to identify patient risk levels and communicate transition plans effectively.
  • Manage a caseload efficiently, demonstrating strong time management skills.
  • Maintain knowledge of utilization management and care coordination processes to support transition planning.
  • Document all interactions and decisions accurately to meet compliance and quality standards.
  • Participate in on-call assignments as required.
  • Ensure current licensure and hospital credentialing are maintained.

Qualifications:

  • Bachelor's degree in Nursing or an Associate's degree with relevant experience.
  • Current RN license specific to the state of employment.
  • Certification in Case Management or the ability to obtain it within a specified timeframe.
  • Extensive clinical experience in managing acutely ill patients.
  • Experience in managed care or case management settings.
  • Proficient in Microsoft Office applications.
  • Strong interpersonal and critical thinking skills.

Preferred Qualifications:

  • Experience with psychiatric and geriatric populations.
  • Bilingual proficiency in English and Spanish.
