Job Title: Home Health Occupational Therapist

Job Summary:

We are seeking a highly motivated and experienced Occupational Therapist to join our home health team. The ideal candidate will have a strong background in occupational therapy and excellent communication skills. As a Home Health Occupational Therapist, you will work closely with patients, families, and healthcare teams to develop and implement personalized care plans that promote independence, mobility, and overall well-being.

Responsibilities:
  • Conduct thorough assessments of patients' physical, emotional, and cognitive abilities to identify areas for improvement.
  • Develop and implement individualized care plans that address patients' unique needs and goals.
  • Provide occupational therapy services to patients in their homes, including exercises, activities, and education to promote independence and mobility.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • 1 year of experience in skilled home health as an Occupational Therapist.
  • Current licensure and certification as an Occupational Therapist.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
Preferred Qualifications:
  • Epic experience.
  • Oasis experience.
Work Environment:

As a Home Health Occupational Therapist, you will work in a fast-paced and dynamic environment that requires flexibility and adaptability. You will be responsible for traveling to patients' homes, which may involve occasional long drives and varying weather conditions.
