The African American Family Outreach Project (AAFOP) is a pioneering initiative dedicated to supporting African American family caregivers of individuals facing mental health challenges and/or substance use disorders. AAFOP offers educational programs, advocacy, and peer support tailored to the unique needs of family caregivers. The Family Support Case Manager (CM) will respond to inquiries from the dedicated warmline, providing essential case management services to families supporting loved ones with mental health conditions. Utilizing a family-driven approach, the CM will offer culturally sensitive support during crises, assist families in completing necessary forms, and provide resources and referrals to appropriate services. The CM will also co-facilitate a monthly support group for African American family members and caregivers, ensuring they receive the necessary support and guidance.
ESSENTIAL JOB DUTIES
Under the supervision of the AAFOP Program Director, the Case Manager's responsibilities include, but are not limited to:
- Responding to and logging calls received on the warmline for African American family members/caregivers
- Conducting needs assessments and developing family-driven care plans as necessary
- Providing case management services that connect families to community resources
- Offering logistical support for AAFOP workshops and events
- Building and maintaining positive relationships with African American service providers for effective referrals
- Coordinating care with hospital/clinic staff to ensure culturally appropriate options are included in care plans
- Conducting outreach events targeting the African American community to discuss mental health issues and gather community input
- Engaging with community leaders to address mental health concerns within the African American community
- Personal experience as a family caregiver for someone with a mental health condition
- Bachelor's Degree (preferably in social sciences)
- At least 2 years of experience in case management
- Experience working with the African American community
- Proficiency in MS Office 365 applications, including Outlook, Word, Excel, and Teams
- Strong written and verbal communication skills
- Valid driver's license and access to a reliable vehicle
- Proof of full Covid-19 vaccination and booster shots
- Successful completion of a fingerprint background check
- Experience supporting clients during crises
- Understanding of Serious Mental Illness (SMI)
- Familiarity with the Alameda County Behavioral Health Care System
- Experience using virtual platforms for service delivery
COVID CONSIDERATIONS: All staff must provide proof of Covid-19 vaccination and booster shots. Air purifiers are utilized in the office.
SALARY: $29.81 per hour (approximately $62,000 annually). Eligible for a pay increase after the introductory period.
