Registered Nurse for Hospice Care

We are seeking a dedicated Hospice RN Case Manager to provide exceptional care in a supportive environment. This role offers a Monday to Friday work schedule, mileage reimbursement, and a comprehensive benefits package including 401K retirement, medical, dental, and vision coverage.

Position Overview:
The Hospice RN Case Manager will primarily serve the designated metro area, working from 8:30 AM to 5 PM, with a manageable patient load of 14 to 16 patients per week.
Key Responsibilities:
- Manage patient visits and on-call schedules, encompassing all phases of the nursing process: assessment, planning, implementation, and evaluation.
- Deliver hospice nursing care based on thorough assessments focused on pain management and symptom control.
- Coordinate care by communicating effectively with attending physicians, hospice staff, and other agencies as necessary.
- Maintain accurate and timely documentation in accordance with agency policies and Medicare regulations.
- Administer medications in compliance with federal and state laws and hospice policies.
- Conduct initial assessments to determine patient and family home care needs.
- Anticipate end-stage disease processes and implement appropriate palliative care measures.
- Provide counseling to patients and families to address end-of-life needs.
- Complete comprehensive death notes when required.
