Hospice Registered Nurse Case Manager

About the Role:
We are seeking a dedicated and compassionate Hospice RN Case Manager to provide exceptional care in a supportive environment. This position offers a Monday to Friday work schedule, along with benefits such as mileage reimbursement, a 401K retirement plan, and four weeks of paid time off.

Job Responsibilities:
The Hospice RN Case Manager will be responsible for:
- Managing a patient caseload of 14 to 16 patients weekly, ensuring high-quality care
- Conducting comprehensive assessments, planning, implementing, and evaluating nursing care
- Providing specialized hospice nursing care focused on pain management and symptom control
- Coordinating with physicians, hospice staff, and other agencies to optimize patient care
- Maintaining accurate and up-to-date documentation in accordance with agency policies
- Administering medications following all applicable laws and regulations
- Collaborating with the Medical Director to assess patient eligibility for hospice services
- Performing initial assessments to identify home care needs for patients and families
- Anticipating end-stage disease processes and implementing palliative nursing procedures
- Offering counseling to patients and families to address end-of-life needs
- Completing comprehensive death notes when necessary
