Job Title: Registered Nurse - Home Health

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in Orange, New Jersey.

Job Summary:

We are looking for a dedicated and experienced RN to provide high-quality nursing care to patients in their homes. As a member of our team, you will work closely with patients, families, and healthcare providers to develop and implement individualized care plans that promote optimal health and well-being.

Responsibilities:
  • Assess and evaluate patients' needs, developing and implementing personalized care plans to address their physical, emotional, and social needs.
  • Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress, updates, and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices and evidence-based care.
Requirements:
  • Current RN licensure in the state of New Jersey.
  • 1+ year of experience in home health or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
