Job Summary
The Southcentral Foundation (SCF) is currently seeking a highly motivated and detail-oriented individual to fill the role of Healthcare Navigator. This position is integral to the success of our organization, as it involves coordinating care for customer-owners who meet the requirements for the Alaska Home and Community Based Waiver Programs.

Estimated Salary Range
$28.80 - $38.41 per hour, with opportunities for growth and advancement based on qualifications and experience.

About the Role
This position requires a strong understanding of healthcare systems, excellent communication skills, and the ability to work effectively with diverse populations. The ideal candidate will be responsible for navigating customer-owners through the complex healthcare system, ensuring they receive the necessary care and support.

Key Responsibilities

  • Navigate customer-owners through the complex healthcare system, ensuring they receive the necessary care and support
  • Develop and implement care plans that address the unique needs of each customer-owner
  • Maintain accurate records and documentation, adhering to SCF's policies and procedures
  • Collaborate with healthcare providers, community agencies, and other stakeholders to ensure seamless care coordination

Requirements
To be successful in this role, candidates must possess:

A Bachelor's degree from an accredited college or university in social work, psychology, rehabilitation, nursing, or a closely related human services fieldThree years of experience as a certified Care Coordinator, maintaining a caseload consisting of at least two recognized waiver typesCertification as a Waiver Care Coordinator, obtained within four months of hireHeart Saver/Basic Life Support certification, acquired within 60 days of hireValid Alaska driver's license, with zero violations in the past five years
