Key Responsibilities- Develop and implement individualized treatment plans in collaboration with the treatment team.
- Provide ongoing support, guidance, and role modeling to program residents, promoting independence and goal achievement.
- Assist with medication management, financial tasks, and community integration efforts.
- Document progress towards treatment goals and communicate effectively with the treatment team.
- Participate in treatment meetings, including weekly staff meetings, and contribute to the development of treatment plans.
- Perform interventions with clients to address identified goals and promote skill development.
- Link clients with other program staff and service providers to ensure comprehensive support.
- Monitor and document medication administration, ensuring a safe and healthy environment for clients.
- Ensure a clean, safe, and home-like environment for clients, adhering to divisional expectations.
- Remain alert and attentive throughout scheduled work shifts, prioritizing client safety and well-being.
- Complete all paperwork, reports, and trainings in a timely manner, meeting divisional expectations.
- Attend and actively participate in core training and maintain certification of required trainings (CPR/First Aid, Crisis Management, MAP).
- Provide crisis intervention and access emergency services as needed, prioritizing client safety and well-being.
- Bachelor's Degree or High School Diploma/GED with one year of related experience.
- Excellent interpersonal, judgement, and coaching skills.
- Strong written and verbal communication skills.
- Ability to utilize basic computer applications.
- Desire to thrive in a fast-paced, client-centered, team-oriented environment.
- Must hold a valid driver's license, have access to an operational and insured vehicle, and be willing to use it to transport clients.
- Strongly prefer a candidate with a demonstrated understanding of and competence in serving culturally diverse populations.
