Social Services Professional

Job Summary:
At King's Daughters, we are seeking a skilled Social Services Professional to join our team. As a key member of our healthcare team, you will play a vital role in assisting patients and their families with issues that accompany illness or inhibit recovery and rehabilitation.
Key Responsibilities:
- Conduct individualized psychosocial and discharge planning assessments to identify emotional, social, and environmental factors related to diagnosis, illness, treatment, and/or life situation that impede or promote healthy transitions of care.
- Provide interventions as indicated in the completion of psychosocial and discharge planning assessments, including crisis intervention, education, advocacy, referral management, and/or mediation.
- Collaborate with and provide education to the interdisciplinary treatment team regarding patients' assessed needs, care transition options, and patient choice, providing representation of the social work perspective in care plan development.
- Monitor, evaluate, and document patient progress according to measurable goals described in the individualized patient care plan, ensuring plans remain updated based on the patient's current needs.
Requirements:
- Bachelor's degree in Social Work
- Kentucky State Licensure as Licensed Social Worker (LSW)
Working Environment:
This role operates in a professional office environment, routinely using standard office equipment such as computers.
