Job Overview

About the Role
We are looking for a compassionate and detail-oriented CNA to assist with patient care, including vital sign checks, personal care, and mobility assistance. Key Responsibilities
- Provide direct patient care, including vital sign checks, personal care, and mobility assistance
- Assist with feeding, bathing, and toileting as needed
- Report any changes or concerns about patient condition to nursing staff
- Collaborate with interdisciplinary team to develop and implement individualized patient care plans

Educational Requirements
- Completion of a state-approved Certified Nursing Assistant training program
- Current certification as a Certified Nursing Assistant in the state of Vermont
- Ability to lift 25 pounds or more
