About the Job

The Clinical Social Work Specialist will be responsible for assessing the psychosocial status of patients and families/caregivers related to the patient's medical condition and environment. They will communicate findings to the registered nurse and other members of the interdisciplinary group, providing an assessment in the patient's identified residence when necessary.

Main Responsibilities

  • Assess the psychosocial status of patients and families/caregivers related to the patient's medical condition and environment;
  • Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, risk for suicide, neglect or abuse and plans intervention based on evaluation findings;
  • Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties;
  • Provide direct counseling services to relieve acute situations;
  • Maintain clinical records on all patients referred to social work;
  • Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs;
  • Serve as liaison between patients and families/caregivers and community agencies;
  • Maintain collaborative relationships with organization personnel to support patient care;
  • Maintain and develop contacts with public and private agencies as resources for patients and personnel;
  • Participate in the development of the individualized plan of care, involving the patient and family;
  • Actively participate in quality assessment performance improvement teams and activities;
  • Participate in in-service education programs as directed;
  • Participate in discharge planning;
  • Perform Other duties as delegated by the Clinical Supervisor.

Requirements

To be successful in this role, you will need:

  • Bachelor's Degree in Social Work or related major, or equivalent combination of education and experience;
  • Competency in working with culturally sensitive support to people from diverse ethnic, cultural, and socioeconomic backgrounds;
  • At least one year in a healthcare setting, preferably in a hospice;
  • Valid LMSW or LCSW license;
  • Driver's License, Car Insurance, Car Registration;
  • Excellent interpersonal and communication skills;
  • Ability to work effectively with individuals and groups;
  • Experience and competency to work in the home or facility setting;
  • Must be able to handle multiple tasks, be flexible, and prioritize workload;
  • Must be highly organized.
