Care Coordination Specialist

We are seeking a dedicated Care Coordination Specialist to join our team at Safe At Home Service Facilitation, LLC. In this role, you will play a crucial part in ensuring the well-being of our clients by conducting face-to-face assessments, completing necessary paperwork, and collaborating with healthcare professionals.
About the JobAs a Care Coordination Specialist, you will be responsible for:
- Conducting thorough face-to-face assessments with clients who have received the long-term services and support waiver.
- Completing required DMAS 99 and DMAS 97AB forms and submitting them to the managed care organization (MCO).
- Performing quarterly face-to-face visits to ensure that agency personal care aides (PCAs) are following established care plans.
- Managing a caseload of patients in your assigned area, supervising PCAs, addressing concerns, and providing information on benefits.
- Evaluating patients' social needs and connecting them with available resources.
- Collaborating with the patient care team (MD, MCO Care Coordinator, CSB) to promote optimal health outcomes for clients.
- Completing other case management responsibilities as needed.
To succeed in this role, you must have:
- An active unrestricted RN license in Virginia.
- CPR certification.
- A valid driver's license in Virginia.
- Previous experience in case management with MCOs is highly desirable.
- Knowledge of Medicaid waivers is an asset.
