Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Sevita, a leading provider of home and community-based specialized healthcare services. As a LPN, you will play a vital role in delivering high-quality care to individuals with intellectual and developmental disabilities, acquired brain injuries, substance abuse issues, and/or emotional or behavioral challenges.

Job Summary:

The Licensed Practical Nurse will provide prescribed medical treatment and personal care services under the direction of a registered nurse or physician. This includes conducting health assessments, providing input on health diagnoses, and developing nursing care plans. The LPN will also be responsible for monitoring and administering medications, tracking and maintaining logs for testing, and providing training to staff members and individuals served.

Responsibilities:
  • Provide basic care and medical treatment to individuals served
  • Conduct health assessments and provide input on health diagnoses
  • Develop nursing care plans and follow-up on physician orders
  • Monitor and administer medications, and track and maintain logs for testing
  • Provide training to staff members and individuals served on topics such as medication administration, universal precautions, and infection control
  • Participate in the on-call system as needed, and may be required to be accessible after hours for emergency coverage
Requirements:
  • Associate Degree in Nursing or related field, or a graduate of an accredited LPN program
  • One year of experience working with the population served in a nursing capacity
  • Current state LPN license and other certifications as required by state licensing regulations
  • Current driver's license, car registration, and auto insurance if providing transportation to individuals receiving services
  • Strong communication skills, with the ability to build rapport with others
  • Self-motivated and detail-oriented, with the ability to multi-task
  • Reliable, responsible, and caring nature, with a commitment to the company's mission and values
