Job Summary

We are seeking a highly skilled and dedicated Case Manager to join our team at Family Services Association. The successful candidate will be responsible for providing case management services to individuals and families, including assessing needs, developing care plans, and coordinating services.

Key Responsibilities:
  • Provide direct service treatment, including diagnostic intakes, needs assessment, and on-going care planning to individual, family, and group members.
  • Implement in-home evidence-based parenting/family strengthening curriculums.
  • Manage caseload, including information/referral advocacy, community outreach, and presentations.
  • Generate and implement Individual Service Plans and other case planning activities.
  • Participate in program start-up activities and subsequent statistical and recording activities.
Requirements:
  • 1-2 years' case management experience in a community-based setting.
  • Experience with providing parenting classes/programs preferred.
  • Bilingual/Spanish speaking preferred.
  • Excellent written and verbal communication skills.
  • Ability to analyze and interpret information to case plan appropriate interventions.
Education and Training:
  • B.A. in social work or related field.
  • Valid CA Driver's License and proof of Automobile Insurance.
Physical and Mental Demands:
  • Working at a desk for extended periods of time with computer/keyboard work.
  • Ability to lift/carry at least 30 pounds.
  • Ability to perform tasks that require bending/stooping.
  • Ability to perform under high pressure/problem-solving situations.
