Case Manager RN

As a Case Manager RN, you will work collaboratively with a panel of physicians to manage patients' specialized needs. This role requires a strong understanding of chronic disease management and the ability to develop and implement effective care management plans.
Key Responsibilities- Evaluate and identify members' needs, working closely with primary care physicians, specialists, and other disciplines to develop case management plans.
- Monitor and evaluate the effectiveness of case management plans, making adjustments as necessary.
- Coordinate interdisciplinary care, including utilization management, transfer coordination, discharge planning, and obtaining authorizations and approvals for outside services.
- Act as a clinical liaison with outside agencies, such as County CCS, non-plan facilities, and third-party administrators.
- Prepare reports, communicate program changes to staff, and develop protocols in accordance with state regulations.
- Advocate for patients, educating them on their condition and empowering them to take responsibility for their care.
- Develop individualized patient education plans, delivering education specific to disease states and facilitating patients' return to normal daily activities.
- Consult with internal and external physicians, healthcare providers, and discharge planners regarding continued care or hospitalization.
- May need to facilitate transportation and housing arrangements for patients, coordinating transmission of clinical and benefit information to patients, families, and outside agencies.
- Participate in data collection and analysis of clinical outcomes and customer satisfaction standards.
- Minimum two years of clinical experience in Oncology.
- Bachelor's degree or equivalent experience required.
- Registered Nurse License (California) and Basic Life Support certification required.
- Demonstrated ability to utilize written and verbal communication, interpersonal, critical thinking, and problem-solving skills.
- Computer literacy skills required.
- Cancer Program: OCN certification required within one year of hire.
- Cancer Program: Two years of experience in Ambulatory Oncology.
- Case Management Certification or certification in the area of specialty preferred.
- Bachelor's degree in nursing or healthcare-related field preferred.
- At least two years of Hematology/Oncology experience preferred.
- Some Hospice and/or palliative care experience preferred.
