Job Summary

We are seeking a skilled Case Manager to join our team at Family Services Association. As a Case Manager, you will play a critical role in providing support and services to our clients, ensuring they receive the care and resources they need to thrive.

Key Responsibilities:

  • Manage a caseload of senior homebound clients, providing regular assessments and care planning to meet their unique needs.
  • Conduct written, in-home, and phone assessments of clients, maintaining confidentiality and adhering to program requirements and HIPAA guidelines.
  • Coordinate resources and referrals for clients, ensuring they receive the necessary support and services.
  • Assist office personnel with data entry and tracking, maintaining accurate records and reports.
  • Report suspicions of elder abuse or neglect to supervisors and assist with the reporting process.
  • Participate in Continuous Quality Improvement (CQI) processes to enhance department and agency success.
  • Attend community events and engage in outreach activities to promote our services and build relationships with community partners.

Requirements:

  • Bachelor's degree in Social Services or equivalent work experience in a social service field.
  • One year of experience in a social service setting, preferably with a focus on case management.
  • Bi-lingual (Spanish speaking and writing) candidate preferred.
  • Background Fingerprint Clearance (Live SCAN) and Drug Test Clearance required.
  • Ability to work on weekends as needed and maintain a valid California Driver's License and Proof of Automobile Insurance.

Physical and Mental Demands:

  • Ability to lift and carry up to 30 pounds safely.
  • Work in a fast-paced environment, managing multiple priorities and tasks.
  • Ability to deal with the public in a professional manner and work with a diverse client population and agency workforce.
