This role focuses on providing high-quality nursing care services in accordance with physician orders, while also serving as a patient advocate and navigator through the healthcare system.
Key Responsibilities- Assess and facilitate coordination of multidisciplinary care across the care continuum.
- Provide communication, advocacy, and education in a culturally competent manner.
- Triage new patient referrals to ensure timely access to care.
- Conduct post-discharge follow-up phone calls and serve as a liaison and patient advocate in navigating the healthcare system.
- Provide patient education and support utilizing knowledge of adult learning competencies and education resources to support complete, connected, and comprehensive care.
- Collect and report data from patients on barriers, hospital experience, healthcare, medications, prior treatments, etc.
- Collaborate with multidisciplinary care teams and expedite the sequence of patient care.
- Serve as a liaison between patients and providers/clinicians to address patient needs.
- Assess patient needs and ensure such needs are integrated into patient care plans.
- Encourage and empower patients to actively participate in decision-making about their treatment and care.
- Massachusetts RN licensure.
- 3 years of RN experience or 5 years of specialty LPN experience.
- Current Basic Life Support (BLS) certification.
- Bachelor of Science in Nursing (BSN) preferred.
