Job Summary:

We are seeking a skilled Occupational Therapy Assistant to join our team at Eden Health. As an Occupational Therapy Assistant, you will work closely with patients in their homes to provide personalized care and support.

Responsibilities:
  • Implement treatment plans ordered by physicians to meet the needs of patients on your caseload.
  • Review caseload with supervising OT on a regular basis to ensure best practices and state guidelines are followed.
  • Evaluate patient assessment findings and communicate relevant information to other healthcare team members in a timely manner.
  • Guarantee that changes in patients' clinical status are communicated to other healthcare team members, necessary office staff, and physicians as per Agency policy.
  • Deliver occupational therapy modalities appropriate to the patient's clinical status and needs, and in accordance with the standard of care for patients' age.
  • Provide clear, concise documentation in medical records as it relates to reimbursement guidelines and Agency policy and procedure.
  • Assess and reassess pain, utilizing appropriate pain management techniques, and educate patients and families regarding pain management.
  • Participate in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  • Recommend referrals to other disciplines and/or community resources as needed.
  • Obtain necessary supplies and equipment to provide care.
  • Support cultural practices that do not harm the patient/family or interfere with the planned course of medical treatment.
  • Teach, supervise, and counsel patients, caregivers, and family members regarding nursing care needs.
  • Participate in the agency Quality Assurance Performance Improvement (QAPI) program.
  • Identify risk of injuries or threats to life and health in the patient's environment and implement appropriate interventions to reduce those risks.
  • Maintain current knowledge of community resources.
Requirements:
  • Meets all of the following: Is licensed or otherwise regulated, if applicable, as an occupational therapy assistant by the state in which practicing, unless licensure does apply.
  • Graduated after successful completion of an occupational therapy assistant education program accredited by the Accreditation Council for Occupational Therapy Education, (ACOTE) of the American Occupational Therapy Association, Inc.
  • Is eligible to take or successfully completed the entry-level certification examination for occupational Therapy assistants developed and administered by the National Board for Certification in Occupational Therapy, Inc.
