Responsibilities:- Develop and maintain relationships with residents and their families, providing personalized care and support.
- Assess and evaluate resident clinical conditions, documenting findings and notifying providers as needed.
- Collaborate with the care team to update and modify care plans, ensuring residents receive the best possible care.
- Administer medications accurately and timely, following established protocols.
- Participate in meetings and reviews to discuss resident outcomes and care.
- Respond to resident, family, or responsible party inquiries in a timely and professional manner.
- Contribute to the review of clinical indicators, such as falls, wounds, and nutrition.
- Competitive hourly rate up to $35.00, including shift differentials.
- Part-time evening and night shift positions available.
