Job Description:

As a County Support Coordinator, you will work closely with clients, families, and community providers to develop and implement personalized support plans. Your primary responsibility will be to facilitate access to community-based services and resources, ensuring that our clients receive the highest level of care and support.

Responsibilities:

  • Develop and implement personalized support plans in collaboration with clients, families, and community providers
  • Facilitate access to community-based services and resources
  • Monitor service delivery and assess client progress
  • Making recommendations for improvement
  • Document all interactions and updates in client records

Requirements:

We require a Bachelor's Degree and at least 1 year of experience working with individuals with developmental disabilities. Strong communication and interpersonal skills are essential, as well as proficiency in Microsoft Office and ability to learn new software quickly.

The ideal candidate will possess excellent time management and organizational skills, with the ability to prioritize multiple tasks and meet deadlines. A personal laptop and reliable transportation are required for this position.

Salary: The estimated salary for this position is $60,000 per annum, commensurate with experience.
