Population Health Care Coordinator

POSITION SUMMARY:
The Population Health/Care Management Assistant identifies patients eligible for Chronic Care Management under the Medicare (or other payer) guidelines. Once identified, the Coordinator contacts the patient by telephone, introduces the program(s) and schedules or enrolls patients with the appropriate program or with a provider if needed. They may also provide education on virtual health platforms and test platforms with patients prior to their appointment time. Coordinators mail out Health Risk Assessments, referrals or other patient information as requested. This position may also assist in message management from incoming calls and be assigned to other special projects if time allows. Provides supportive role to the case management department. This may include but not limited to the gathering and transmission of data to ensure appropriate patient outcomes. Interaction with the various stakeholders involved with coordinating care of patients. Strong communication, administrative and customer service skills are essential.
EDUCATION/TRAINING:
High School degree plus some college or medical field training. Must be knowledgeable of healthcare insurance rules. Medical Assistant (MA, PCA, CNA) experience preferred.
LICENSURE/CERTIFICATION: MA, PCA, CNA certification preferred.
EXPERIENCE/SKILLS/ABILITIES: Must be able to type accurately and the ability to type 35 wpm is preferred. Excellent verbal, written, organizational and critical decision-making skills required. Must be able to read, write legibly and communicate effectively. Ability to handle multiple tasks simultaneously. Good interpersonal skills and ability to deal with all levels of staff within the organization and community under varied conditions. Ability to operate a variety of general office equipment and use of a computer. Successful completion of annual job specific competencies and skill verification tools required.
