Job SummaryWe are seeking a skilled Nurse Practitioner/Physician Assistant to join our team as a PRN/Part Time provider. As a member of our team, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
Responsibilities- Conduct home visits to assess patients' health and provide medical care
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care
- Provide health education and counseling to patients and their families
- Document patient information and communicate with the healthcare team
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP or PA licensure in the state of practice
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner/Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
